If you have insurance coverage, I will be happy to process your insurance claim for you as a courtesy. To do this, I will need complete insurance information from you. Please click the link at the bottom of this page to go to the insurance information page.  If your insurance should change in the future, please let me know immediately as well as provide the updated information. Coverage must be active at the time of service.

If I am not contracted with your insurance company ("out-of-network") there is still a good chance a significant portion of my fee will be covered. Typically, out-of-network benefits pay less (you will have a higher co-pay and/or co-insurance and may also have a separate deductible), but in most cases 50-70% of the cost is covered. Group Health “Options” has out-of-network coverage, but Group Health (HMO) does not. As a courtesy, I will bill your insurance provider for you unless out-of-network providers are not able to submit claims directly in which case you are required to pay for services and I will provide you with a “super-bill” which you will submit for reimbursement.

As a health care provider, my relationship is with you, not your insurance company. While the filing of insurance claims is a courtesy that I extend to my patients, all charges are ultimately your responsibility. This includes any copays, coinsurance, and deductibles.
